Topsail Clinical — continuing education for post-acute care

The referral everyone saw coming, sent six months late.

You knew in March. The referral went out in September. One hour of CE on recognizing hospice eligibility while there is still enough time left to do something with it.

What you can use on Monday

Built for any registered nurse who has to decide whether a declining patient goes back to rehab or finally gets a goals-of-care conversation.

Who this is for

Written for any registered nurse who watches a patient decline and has to decide what comes next, not for the hospice team on the receiving end of the referral.

How it works

  1. Enroll

Submit payment. The course opens right away — no waiting on a cohort, no scheduled webinar.

  2. Complete the hour

Read the module (about 60 minutes) — three cases, the two baseline criteria, and the disease-specific indicators reviewers actually look at.

  3. Get your certificate

Score 80% or higher on the 10-item post-test and complete the evaluation. Your certificate is issued immediately with the ONA activity number on it.

Orient your whole case management department in an hour.

Department licensing for hospitals, SNFs, home health and hospice agencies.

Your case managers decide, every day, whether a declining patient goes back to rehab or finally gets a goals-of-care conversation. Hospice eligibility criteria are not in anyone's orientation — not in nursing school, not at hire — which makes it one of the few gaps in your organization that closes in an hour.

A department license lets you assign this as onboarding for new case management staff, or as annual education for the whole team. Each learner takes it on their own time, passes the post-test, and gets their own certificate with the ONA activity number on it — so it counts toward licensure for them, and it goes in your education file as documentation for you.

Each seat is one of your own employees. They get their own login, their own post-test, and their own certificate. Hospice and home health agencies: this works the same way for your liaisons, admissions nurses and IDG members — your staff, your license.

We do not sell seats to a provider to hand out free to its referral sources. A free contact hour has real value to a licensed clinician who has to buy CE anyway, and regulators treat that value as remuneration — see OIG Advisory Opinion 22-14. So we sell to the organization that employs the learner, or to the learner directly. It is the version that holds up under review.

One contact hour, covering the two baseline criteria and the decline indicators for eight referral diagnoses.

Late referrals are rarely a knowledge failure at the hospice end. Hospice eligibility criteria live in the Medicare benefit rather than in any orientation curriculum, so the window narrows before anyone is taught what it looks like.
