Topsail Clinical — compliance tools for hospice and skilled nursing
The surveyor accepts the plan and leaves. What happens next is usually whoever remembers, on whatever form they build that morning. The 418.76 POC Monitoring Kit is eight instruments that make the follow-through produce evidence instead of intentions.

42 CFR 418.76 governs hospice aide and homemaker services. In CMS hospice survey data it was the third most frequently cited condition of participation in calendar year 2025, appearing in 30.5 percent of all hospice surveys closed that year, behind only the plan of care at 418.56 and the comprehensive assessment at 418.54.
Most citations turn on the aide care plan and the supervision record: whether a registered nurse assigned the aide and prepared the written task instructions, whether the tasks the aide performed match the plan of care, whether the 14-day supervisory visit happened on time, and whether competency and in-service documentation is complete. A meaningful share involve aide visits or assigned tasks that were not furnished at the frequency the plan of care ordered. Almost none rise to immediate jeopardy, which is precisely why they survive one plan of correction and reappear on the next survey.
The kit assumes you already have a plan of correction. What it gives you is the monitoring underneath it: who checks what, how often, against which standard, and what the record looks like when a surveyor asks you to prove it held.
Eight monitoring instruments, numbered HOS-001 through HOS-008, in a single 18-page Word document. Every instrument is editable, so your agency name, your thresholds, and your review cadence go on it before anyone fills one out.
They are built to run in order: HOS-006 establishes the baseline, HOS-007 sets the monitoring plan with an owner, a frequency, a duration and a threshold, the audit instruments run on the schedule you set, and HOS-008 assembles the evidence for closure.
Topsail Clinical is a remote clinical consulting practice working with hospice and skilled nursing organizations, and with the lenders, owners, and receivers responsible for them. The kit was written by a mock survey consultant who writes the finding the way a surveyor would, and then the correction a surveyor would accept.
This kit is a set of monitoring instruments, not legal advice and not a guarantee of any survey outcome. Your plan of correction, your thresholds, and your regulatory obligations remain yours. Because the kit is delivered as an immediate download, all sales are final.
If the monitoring is the part that keeps slipping, that is the part this fixes.
Eight instruments, one document, $497. You can have it open on your desk in the next five minutes.
